On Track is a vibrant and approachable mental health initiative created for younger adults and clinicians, altering the way we view Measurement-Based Care (MBC). A collaboration with Tiya Wagh and Cathy Chen
With a voice that is welcoming, transparent, and empowering, On Track shifts the perception of MBC from something clinical or tedious to something joyful, personal, and meaningful. Inspired by journaling culture and the satisfaction of tracking progress, the brand introduces a visual language that is both minimal and nostalgic. The use of vibrant, youthful colors and clean design elements speaks to a unisex audience aged 18 to 28. On Track makes mental health tracking feel like a daily act of self-care rather than a task.
Through a punch card reward system, patients are encouraged to stay engaged and celebrate their small wins. The accompanying journal is a key tool that supports mood tracking, habit building, and reflective journaling. This helps users recognize patterns, regulate their emotions, and better understand their mental health.
